In episode 2 of Mary Kills People, a complex and emotional bond begins to form between Woo So-jung (played by Lee Bo-young) and Cho Hyun-woo (played by Lee Min-ki), who has chosen assisted death. While Hyun-woo prepares for his final moments, So-jung struggles with her desire to save him, building emotional tension and hinting at upcoming turmoil.

The episode opens with a flashback: So-jung, her college friend Choi Dae-hyun (Kang Ki-young), and nurse Choi Ye-na (Yoon Ga-yi) form a team to carry out assisted deaths for suffering patients. They reflect on the three conditions for legal assisted dying—irreversible diagnosis by three or more doctors, unbearable physical disability, and pain unmanageable by any medication.

In the present, So-jung and Dae-hyun visit high school student Jo Soo-young (Lee Won-jung), who has only two weeks to live due to stage 4 colon cancer. Soo-young pleads for a quicker death, unable to bear the pain that awaits. Moved, So-jung chooses to prioritize Soo-young’s assisted death over Hyun-woo’s.
Later, So-jung visits a welfare hospital where she coincidentally meets Hyun-woo. Their shared moments reveal growing affection, as Hyun-woo reflects on preparing for the end. When asked why she helps dying people, So-jung says, “I believe people should have the right to choose life or death. Death isn’t a crime.”
Hyun-woo later takes So-jung to the orphanage where he grew up—his happiest place—and expresses his desire to spend his final moments nearby. He explains his choice for assisted death: “I tried everything. Nothing changed. That’s why I could finally give up.”
Desperate to save him, So-jung secretly finds a new clinical trial and brings Hyun-woo there. But he lashes out, accusing her of being just like other doctors who ignore the patient’s pain. After walking away from her, h
e disappears, prompting So-jung to search frantically—from his delivery job to his dormitory—but to no avail.
Just when So-jung is threatened by an abusive husband at the hospital, Hyun-woo reappears and rescues her. As she treats his injured hand, the two reconcile and exchange heartfelt apologies. When Hyun-woo gifts her a hairpin and she tearfully confesses, “I want you to live,” he responds with a gentle kiss.
Meanwhile, detective Boo appears at Hyun-woo’s old dorm and suspiciously pays his overdue rent. In the final scene, as Hyun-woo and So-jung share a tender moment on a crowded street, a completely different man—introduced as Hyun-woo—emerges, leaving viewers shocked and questioning his true identity.
Following the episode, viewers were stunned by the twist:
